Job Title: Case Manager

Join Select Medical's team of dedicated healthcare professionals as a Case Manager, playing a vital role in the rehabilitation process of patients with complex medical conditions.

About the Role:

The Case Manager will assess the social work needs of patients and provide intervention as part of the discharge planning process. This includes:

  • Clinical Interventions/Discharge and Care Planning Management
  • Fiscal Management
  • Payer/Referral Management

The ideal candidate will have a strong background in social work or nursing, with a focus on case management and discharge planning. A valid state licensure in Social Work (LCSW or LSW) or current state licensure as a Registered Nurse (RN) is required.

Responsibilities:

The Case Manager will be responsible for:

  • Coordinating healthcare decisions using a systematic approach to ensure treatment plans improve quality and outcomes
  • Coordinating care across the continuum
  • Promoting cost-effective care within the allotted time frame
  • Ensuring payments for hospital-based services meet patient-related utilization management criteria
  • Implementing safe and appropriate discharge plans
