We are seeking a highly skilled Pharmacist to join our team at Dayton Children's Hospital. As a key member of our Home Infusion Pharmacy, you will be responsible for providing pharmaceutical care to our patients, primarily through intravenous therapy.
Key Responsibilities- Assess and develop care plans for patients, ensuring accurate and appropriate medication therapy
- Consult and coordinate with physicians, nurses, and other healthcare professionals to ensure seamless patient care
- Monitor and document patients' progress and outcomes, making adjustments to care plans as needed
- Compound sterile products and select appropriate supplies for patient care
- Bachelor's Degree in Pharmacy or PharmD Degree
- Ohio State Board of Pharmacy registration as a Registered Pharmacist
