ROLE SUMMARY:

The Case Management Specialist plays a crucial role in enhancing the quality of life and recovery success for our clients. This position focuses on developing personalized support strategies aimed at fostering self-sufficiency and long-term recovery. The process involves assessment, planning, implementation, coordination, monitoring, and evaluation to achieve client-centered goals.

KEY RESPONSIBILITIES:

  • Conduct thorough assessments for new clients and formulate tailored care plans
  • Implement care plans efficiently and consistently
  • Collaborate with interdisciplinary team members across Clinical, Programs, Education, and Aftercare Departments
  • Assist clients in accessing essential medical, social, educational, and vocational resources
  • Facilitate care coordination and information sharing
  • Act as an advocate for clients, guiding them in making informed decisions about their treatment
  • Maintain regular contact with clients to document their progress and ensure successful outcomes
  • Complete necessary documentation including assessments, care plans, notes, and discharges
  • Comply with Kentucky Administrative Regulation (KAR) standards and guidelines

QUALIFICATIONS:

  • Bachelor's degree in behavioral science or a related healthcare discipline
  • At least one year of relevant post-graduate experience in healthcare or human services
  • Strong written and verbal communication abilities
  • Excellent organizational and time management skills
  • Preferred experience as a Targeted Case Manager in the substance use disorder field
  • Familiarity with case management principles and healthcare management
  • Knowledge of Medicaid processes is advantageous
  • Strong problem-solving skills and ability to manage multiple tasks
  • Collaborative mindset with a focus on process improvement
  • Current TCM (CORE/SUD) certification is a plus
