As an End-of-Life Care Specialist, you will join our interdisciplinary team providing patient-centered care. Responsibilities include:
- Conducting assessments and reassessments as needed.
- Developing and implementing participant plans of care.
* Full-time position
* Competitive base salary: $120,000-$150,000 per year (estimated)
* Benefits package including medical malpractice coverage and 403b plan
* Qualifying site for Public Student Loan Forgiveness (PSLF)
