Job Title: Home Health Speech Language Pathologist

Alternate Solutions Health Network LLC is seeking a skilled Home Health Speech Language Pathologist to join our team in Fort Thomas, Kentucky.

About the Job

We are looking for a compassionate and experienced Speech Therapist to provide home-based skilled speech therapy care for patients. As a Home Health Speech Language Pathologist, you will work with patients and their families to develop individualized plans of care and make adjustments as needed.

Responsibilities
  • Develop and implement individualized plans of care for patients
  • Provide home-based skilled speech therapy care for patients
  • Educate patients and their families on therapeutic activities and home exercise programs
  • Observe, record, and report patient reactions, changes, and progress to attending physicians and agency staff
  • Minimize patient risk by teaching preventative measures and making good decisions
  • Counsel patients and families on disease management and provide support
Requirements
  • Current license in the state of employment
  • Minimum one-year experience as a Speech Therapist in an acute care setting
  • Valid driver's license and auto insurance
  • Capable of all physical demands
