Job Title: Home Health Speech Language Pathologist

Alternate Solutions Health Network LLC is seeking a skilled Home Health Speech Language Pathologist to join our team in Fort Thomas, Kentucky.

About the Role:

We are dedicated to providing high-quality care to our patients in the comfort of their own homes. As a Home Health Speech Language Pathologist, you will work closely with our interdisciplinary team to provide compassionate and effective speech therapy services to patients with a range of conditions.

Responsibilities:
  • Provide home-based skilled speech therapy care for patients as directed by the attending physician.
  • Guide patients in the use of therapeutic, creative, and self-care activities to improve functioning.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
Patient Outcomes:

Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication, and/or any other speech therapy-related function.

Minimize Patient Risk:

Help decrease re-hospitalizations by teaching patients on preventative measures and making good decisions.

Family Educator/Advocate:

Counsel patients and families on the disease/injury and how to manage in all phases of speech therapy.

Policies:

Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.

Operations:

Attend in-service training and mandatory agency meetings.
