Job Summary

We are seeking a highly skilled Outpatient Specialty Pharmacist to join our team at Community Memorial Health System. As a key member of our pharmacy team, you will provide pharmaceutical services supporting all outpatient activities with a focus on Specialty Pharmacy.

Responsibilities
  • Provide prescription processing, patient counseling, and technician oversight in a fast-paced outpatient setting.
  • Enroll patients in our Patient Management Program to create personalized disease state and specialized drug care plans.
  • Manage performance improvement and quality improvement plans for Specialty Pharmacy adherence.
  • Perform all duties conforming to CMHS organizational standards and regulatory requirements.
Requirements
  • Current California Pharmacist License.
  • Three (3) years of experience in outpatient pharmacy.
  • Certified Specialty Pharmacist through National Association of Specialty Pharmacy, or able to obtain within two (2) years from hire date.
Preferred Qualifications
  • Master's degree (M.A.) or Doctor of Pharmacy Degree (Ph.D.).
  • Experience with specialty pharmacy accreditation (URAC, ACHC, etc.).
  • 340B pharmacy experience.
