Job Description:

We are seeking compassionate and skilled professionals to join our team as End of Life Care Providers. As a member of our team, you will be responsible for delivering high-quality care to assigned patients.

Responsibilities:

  • Plan, coordinate, and provide hospice care in accordance with the patient's plan of care.
  • Coordinate hospice care by working with other team members and with patients/family members to ensure needs are met.
  • Collaborate with registered nurse managers to administer prescribed medications.
  • Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.
  • Take responsibility for ensuring patients' plans of care are kept current.
  • Continually strive to improve nursing care skills by attending in-service education, formal education, workshops, conferences, professional organizations, and individual research.
  • Actively participate in QAPI meetings and activities.
  • Perform death visits per agency policy.
  • Participate in patient discharge planning process.

Qualifications:

  • Must be a graduate from an accredited School of Nursing.
  • Must be licensed in the State as a Registered Nurse.
  • One year of experience as a professional nurse within the last three years or have a baccalaureate degree in nursing from a program accredited by the National League of Nursing.
  • Compassionate and caring demeanor.
  • Excellent written and verbal communication skills.
  • Must have excellent assessment skills and problem-solving skills.
  • Should be skillful in organization and time management principles and knowledge of case management processes.
  • Must be a licensed driver with an insured automobile in good working order.
  • Must have no exceptions to RN license, and be able to administer pharmaceuticals, perform medication reconciliation, and any nursing procedures or special treatments needed within the scope of nursing practice for the state.
