About the Role

We're looking for a skilled healthcare professional to provide high-quality care to our highest-risk patients in assisted living and community-based settings. As a member of our interdisciplinary team, you'll conduct comprehensive assessments, manage patients through acute medical situations, and ensure accurate documentation.

Responsibilities
  • Conduct advanced illness and advanced care planning conversations
  • Provide patients and caregivers with counseling and education
  • Facilitate agreement and implementation of the patient's plan of care
  • Coordinate care as patients transition through different levels of care and care settings
  • Review orders and interventions for appropriateness and response to treatment

Requirements

Preferred Qualifications

We're looking for candidates with 1+ years of clinical experience in senior living, geriatric medicine, or home care. Experience in meeting the healthcare needs of patients with complex behavioral, social, and/or functional needs is also a plus.
