About the RoleWe're looking for a skilled healthcare professional to provide high-quality care to our highest-risk patients in assisted living and community-based settings. As a member of our interdisciplinary team, you'll conduct comprehensive assessments, manage patients through acute medical situations, and ensure accurate documentation.
Responsibilities- Conduct advanced illness and advanced care planning conversations
- Provide patients and caregivers with counseling and education
- Facilitate agreement and implementation of the patient's plan of care
- Coordinate care as patients transition through different levels of care and care settings
- Review orders and interventions for appropriateness and response to treatment

RequirementsTo be considered for this role, you'll need to have an active and unrestricted Nurse Practitioner or Physician Assistant license in the state where you reside, or the ability to obtain one by start date. You'll also need to have a graduate degree from an accredited program and relevant certification.
Preferred QualificationsWe're looking for candidates with 1+ years of clinical experience in senior living, geriatric medicine, or home care. Experience in meeting the healthcare needs of patients with complex behavioral, social, and/or functional needs is also a plus.
