Certified Home Healthcare Nurse Specialist

Job Summary:
We are seeking a skilled Certified Home Healthcare Nurse Specialist to join our team at Henry Ford Health System.
About the Role:
This is a part-time day shift position, offering an estimated annual salary of $65,000-$75,000, depending on experience and qualifications.
Key Responsibilities:
- Deliver high-quality patient care in home settings, working under general supervision from a nursing supervisor.
- Assess patient needs and coordinate with other healthcare professionals to ensure comprehensive care.
- Develop and implement individualized care plans to meet patients' unique needs.
- Maintain accurate and detailed records of patient interactions and care provided.
- Collaborate with interdisciplinary teams to achieve optimal patient outcomes.
Requirements:
- Graduation from an accredited Registered Nurse program required.
- Current licensure as a registered nurse in the State of Michigan is essential.
- A minimum of 9 months to 1 year of clinical nursing experience preferred.
- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
- Strong computer skills, including electronic documentation of patient care.
- Possession of a valid driver's license, satisfactory driving record, and reliable transportation.
- Able to lift, carry, transport, and move clients as needed.
