Nurse Practitioner

Mass General Brigham Healthcare at Home is seeking a skilled Registered Nurse (RN) to provide high-quality, clinical services to patients in various settings within the community. As a member of our interdisciplinary team, you will deliver patient care that meets the organization's philosophy, policy, and standards of community health practice.
The ideal candidate will possess a strong background in acute care nursing, with a minimum of one year of experience. Prior home care experience is preferred, but not required. You will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing education to patients and their families.

Responsibilities- Provide quality, clinical services to patients in various settings within the community
- Assess patients' needs and develop individualized care plans
- Implement care plans and provide education to patients and their families
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Document patient information accurately and timely
- Graduate of an approved School of Nursing
- Current Massachusetts license as Registered Nurse
- Minimum of one year previous nursing experience in acute care setting
- Prior home care experience preferred
- Ability to work with various computer software and knowledge of hand-held patient care computer devices preferred
