Experienced Case Management Professional
Minimum of 2 years of experience in a clinical case management role
At least 2 years of post-master's experience
2+ years of experience in community health or social services environments
Experience with vulnerable and at-risk populations is essential
Familiarity with outreach initiatives and community resources is advantageous.

Must possess strong interviewing skills to effectively gather necessary information from clients.

Must be an attentive listener, capable of comprehending the nuances of client communications.
Must be a keen observer, able to accurately document client behaviors and needs.
Must be an effective negotiator, able to establish clear agreements with clients and service providers.
Ability to collaborate effectively within interdisciplinary teams.
Proficient in Microsoft Office 365 suite (Word, Excel, PowerPoint, Teams)
Ability to prioritize tasks and solve problems while managing a caseload.
Excellent oral and written communication skills are essential.
Strong interpersonal skills are required.
Knowledge of social services, human behavior, psychopathology, and DSM is necessary.
Exceptional organizational skills are a must.

RESPONSIBILITIES:


Engages with diverse populations facing complex needs; assisting clients in adhering to established treatment plans, enhancing social functioning, and improving overall quality of life.

Acts as a planner, advocate, broker, and record keeper for clients.

Initiates outreach to educate clients about available social services to improve compliance with medical treatment and case management plans.

Employs a strength-based, person-centered approach and motivational interviewing techniques to foster rapport and engage clients in services aimed at overcoming barriers to care.

Develops a therapeutic alliance with clients.

Evaluates clients for needs related to addictions, mental health issues, homelessness, domestic violence, pregnancy, family challenges, and other social determinants affecting healthcare.

Assists clients in addressing social needs that influence their healthcare.
Creates and maintains a person-centered case plan with clients to ensure services are tailored to individual needs.
Guides and supports clients to enhance their adherence to medical and behavioral health treatments.

Provides advocacy, referrals, and connections to community resources and support networks to meet identified needs, including assistance with health insurance enrollment.

Conducts regular follow-ups with clients to ensure successful access to and utilization of community resources or in-house services.
Offers crisis intervention as needed.

Collaborates with a multidisciplinary team to share information regarding client status and to address challenging patient situations.

Accurately documents all client interactions on appropriate social service forms; submits progress notes and encounter forms within established timelines.
Assesses and assists with insurance and other social service benefits as necessary.
Facilitates counseling and psycho-educational groups as required.
Completes psychosocial assessments as needed.
Works with team members and providers to compile medical summary reports for benefits as necessary.
Conducts mental status evaluations as necessary (based on licensure level)

Provides brief and solution-focused therapy related to medical care and access to services (based on licensure level).

Participates in case conferences as required.
Trains peers and health center staff as needed.
Engages in continuing education to maintain licensure and professional standards.
Adheres to the Code of Ethics for the relevant discipline.
Maintains professional relationships and appropriate boundaries with clients.
Participates in professional development opportunities and regular supervision.
Attends departmental, health center, and agency meetings, activities, and events.
Performs other duties as assigned.
