SHIELDS' mission focuses on developing, delivering, and assessing culturally aware, comprehensive service models that empower and advocate for high-risk families in South Los Angeles.

The Plans of Safe Care (POSC) Navigator will conduct hospital and home visits for prenatal and postpartum women identified with substance misuse or a substance use disorder. Navigators will support families, perform assessments, create Plans of Safe Care, and provide education on prenatal and postnatal care, breastfeeding, parent-infant bonding, child development, home safety, and other maternal and child health topics. They will also identify participant strengths, needs, barriers, and risks, linking families to behavioral health, home visitation, and other community resources. Additionally, the Navigator will offer empathetic support and feedback while working with new mothers, infants, and family members. This role is part of a county-wide collaboration and will involve data collection, meetings, and training related to the broader implementation of Plans of Safe Care in Los Angeles County.

Key Responsibilities:

  • Collaborate closely with hospital social work teams and medical staff.
  • Conduct assessments and enroll participants at the bedside.
  • Develop a Plan of Safe Care (POSC) in partnership with families and hospital teams for all enrolled families.
  • Effectively perform home visits with prenatal or postpartum participants.
  • Be available to respond to participants during non-working hours as needed.
  • Build trust and rapport with participants, providing support through an empathetic and family-centered approach.
  • Complete all necessary documentation, including screenings, assessments, and POSC-related documents, in a timely and organized manner, ensuring clear and well-written progress notes.
  • Educate participants on maternal and infant health, parenting, attachment, home safety, infant behavior, and related topics using a participant and family-centered approach.
  • Facilitate linkages and referrals for participants and their families according to the appropriate level of service; ensure all linkages and referrals are executed.
  • Coordinate support services, including but not limited to substance use disorder treatment, home visitation, and linking participants to auxiliary services.
  • Participate in case conferences and related meetings with partner programs and agencies to coordinate support services for participants.
  • Maintain appropriate service provision with referral sources, such as the Department of Children and Family Services or community agencies.
  • Collect required data and enter it into an electronic database, including updated community resources, participant information, and generate reports.
  • Provide monthly reports to the program manager.
  • Respond to information requests promptly.
  • Maintain positive working relationships with local linkage and referral agencies.
  • Stay informed about existing community programs and resources, their services, and eligibility requirements.
  • Ensure caseload documentation complies with all agency policies and procedures, as well as federal, state, county, and local regulations.
  • Attend and engage in staff meetings, hospital meetings, grant meetings, supervision, and training as required.
  • Assist in coordinating program events and activities.

Position Requirements:

  • Basic advocacy skills.
  • Ability to present ideas, information, and viewpoints clearly, both verbally and in writing.
  • Proficient in using personal computers, including various word processing software.
  • Utilize analysis, experience, and judgment for decision-making.
  • Demonstrate commitment to team objectives and SHIELDS For Families philosophies.
  • Adapt to changing needs by acquiring new skills and knowledge.
  • Current California driver's license with an acceptable driving record.
  • Provide services within a culturally sensitive, ethnic-informed model of practice, respecting the community and participants.
  • Experience with substance abuse and underserved populations is required; SUD certificate preferred.
  • Bachelor's degree required.
  • Bilingual (English/Spanish) required.
  • Experience in maternal health preferred.
  • Access to reliable transportation.
