Responsibilities

The Case Manager is responsible for overseeing the quality and resource management of all patients admitted to our facilities. This role advocates for patients, promoting cost-effective care while ensuring seamless collaboration among healthcare providers. The Case Manager will conduct thorough assessments to ensure the delivery of quality, clinically effective, and cost-efficient care.

Qualifications

Education, Experience, and Training:

  1. A minimum of 5 years of post-graduate experience from an accredited nursing program with a current state Registered Nurse license is required. Alternatively, a minimum of 5 years post-graduate experience from an accredited school of Social Work for Licensed Clinical Social Workers is acceptable, though RN Case Managers are preferred.
  2. Five years of acute care nursing experience is preferred, with at least one year in case management, discharge planning, or nursing management.
  3. Current BCLS (AHA) certification is preferred.
  4. Familiarity with Milliman and InterQual Criteria is preferred.
  5. Basic to intermediate computer skills are necessary.
