Responsibilities
The Case Manager is responsible for overseeing the quality and resource management of all patients admitted to our facilities. This role advocates for patients, promoting cost-effective care while ensuring seamless collaboration among healthcare providers. The Case Manager will conduct thorough assessments to ensure the delivery of quality, clinically effective, and cost-efficient care.
Qualifications
Education, Experience, and Training:
- A minimum of 5 years of post-graduate experience from an accredited nursing program with a current state Registered Nurse license is required. Alternatively, a minimum of 5 years post-graduate experience from an accredited school of Social Work for Licensed Clinical Social Workers is acceptable, though RN Case Managers are preferred.
- Five years of acute care nursing experience is preferred, with at least one year in case management, discharge planning, or nursing management.
- Current BCLS (AHA) certification is preferred.
- Familiarity with Milliman and InterQual Criteria is preferred.
- Basic to intermediate computer skills are necessary.
