Job Summary

Woodlands Home Health is seeking a skilled Physical Therapist to join their team of dedicated professionals. As a Physical Therapist, you will have the opportunity to work with patients in their homes, providing one-on-one care and support to help them achieve their goals.

Responsibilities
  • Evaluate and assess patients' physical conditions and develop appropriate treatment plans
  • Administer therapeutic exercises, manual therapy techniques, and modalities to improve patients' mobility, strength, and function
  • Monitor patients' progress and modify treatment plans as necessary
  • Educate patients and their families on proper techniques for managing their conditions and preventing further injury
  • Collaborate with other healthcare professionals to coordinate patient care and ensure optimal outcomes
  • Maintain accurate and up-to-date patient records
Requirements
  • Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program
  • Must have an active Physical Therapy license in state(s) of practice
  • Ongoing continuing education and professional development to maintain current licensure and certification
