Hospice Admissions RN

The Hospice Admissions RN is a seasoned registered professional nurse who initiates a comprehensive plan of care for patients with life-limiting illnesses. This role functions under the supervision of the DPCS and collaborates with the attending physician, Hospice Medical Director, and other members of the interdisciplinary team.

Responsibilities
  • Provide empathetic and holistic care to patients and their families
  • Conduct thorough patient assessments and develop individualized care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor and manage symptoms, including pain management
  • Educate patients and families on disease management and end-of-life care options
  • Collaborate with the interdisciplinary team to ensure coordinated and patient-centered care
  • Maintain accurate and up-to-date patient records
  • Follow infection control protocols to prevent the spread of diseases
  • Perform coding and documentation in accordance with regulatory standards
  • Provide ostomy care and assist with sterile processing as needed
  • Assist with ventilator management, gastric lavage, and IV infusion when required
Qualifications
  • Valid nursing license in the state of California
  • Minimum of 2 years of experience in hospice, palliative care, home health, or community health nursing
  • Strong knowledge of patient assessment techniques and pain management strategies
  • Familiarity with infection control protocols and standards
  • Proficiency in coding and documentation practices related to hospice care
  • Ability to effectively communicate with patients, families, and interdisciplinary team members
  • Compassionate, empathetic, and patient-centered approach to care
