Job Summary

We are seeking a dedicated and compassionate Healthcare Professional to join our team at 365 Health Services. As a Home Health Aide, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Assist patients with personal care activities, including hygiene, mobility, and elimination, as prescribed in the plan of care.
  • Provide a safe and supportive environment for patients, using equipment safely and properly.
  • Practice Universal Infection Control Precautions in all contacts with patients.
  • Assist with home maintenance activities, including light housekeeping and meal preparation, as prescribed in the care plan.
  • Monitor patients' fluid and nutrition intake, temperature, pulse, respiration, skin, and basic elements of body functioning, and record/report appropriately.
  • Use safe transfer, ambulation, normal range of motion, and positioning techniques.
  • Identify the need for intervention by other members of the home health team and report to the RN or Director of Clinical Services.
  • Respect patients' autonomy and right to confidentiality.
  • Document care provided and response of consumer/family, submitting documentation in a timely manner.
  • Perform within the limits of preparation and experience.
  • Ensure compliance with local, state, and federal laws, Medicare regulations, and established agency policies and procedures.
  • Manage a caseload and deliver care in a cost-effective manner.
  • Meet or exceed 365 Health Services' established service standards.
  • Conduct all business activities in a professional and ethical manner.
  • Use supplies and equipment effectively and efficiently.
  • Complete a minimum of 12 hours of in-service training per year.
  • Maintain regular attendance.
  • Adhere to 365 Health Services' Code of Conduct.
  • Interact with all staff in a positive fashion, supporting 365 Health Services' mission and vision.
