Overview

The LPN is responsible for providing safe and reliable patient care, under the direction and supervision of a Registered Nurse (RN).

We are seeking a skilled and compassionate LPN to join our team in the Acute Rehabilitation Unit. As a member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and achieving excellent outcomes.

Responsibilities
  • Collaborate with the healthcare team to provide patient care and achieve excellent outcomes and patient-centric goals of care.
  • Contribute to the admission assessment process by collecting, reporting, and recording objective and subjective data in an accurate and timely manner.
  • Assist in the identification of appropriate patient care interventions to maintain comfort, support human functions and responses, and maintain an environment conducive to safety and well-being.
  • Implement measures to promote a safe environment.
  • May receive telephone, verbal, and written physician orders and process per hospital policy.
  • Administer medications and perform tasks within the scope of practice.
  • Integrate a multidisciplinary approach to maintain comfort, support human functions and responses, and maintain an environment conducive to safety and well-being.
  • Collaborate with the patient/family to anticipate steps needed to ensure safe transitions in care.
Requirements
  • Completed an accredited LPN program and/or be eligible to take licensure exam.
  • Previous work experience in a rehabilitative setting.
  • Possess current, unexpired LPN licensure in the State of Iowa or compact state.
  • Current BLS certification.
  • Valid driver's license when driving any vehicle for work-related reasons.
  • Completion of IV Therapy certification.
