Pediatric Specialty Care LPN Job Description

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our pediatric care team in Philadelphia, PA. As an LPN, you will play a vital role in ensuring the general health and well-being of our patients by providing direct nursing care under the medical direction and supervision of our attending physicians, RNs, and facility Medical Director.

Responsibilities:
  • Administer medications and treatments to patients as ordered by the physician, utilizing the 5 rights of medication administration.
  • Perform and promote timely and accurate documentation, including care plans, nursing notes, medical administration records, treatment records, and other necessary records.
  • Assist in performing accurate, thorough, and timely completion of nursing admission evaluations and re-evaluations of patient care needs in accordance with Nursing Department standards of practice.
  • Provide evaluation of patient needs and complete individual education in accordance with established procedures.
  • Perform all position duties according to safety rules and standards of practice required for this position, and as required to ensure the general safety of staff, patients, and visitors of the facility.
  • Report to the Director of Nursing, Assistant Director of Nursing, RN Supervisor, and/or licensed professional any unusual symptoms, changes of condition, daily needs, and progress of patients.
  • Communicate with MD/NP/PA for orders or updates according to patient response to treatment plan and/or situation, and update family members as needed.
  • Participate in Change-of-Shift Report and be responsible for bringing nursing personnel of next shift up to date regarding the care and needs of patients.
  • Communicate with RN and the patient's healthcare team members to seek guidance when patient care needs exceed LPN scope of practice or if patient condition deteriorates or patient needs immediate assistance.
  • Attend and participate in orientation and in-service training prior to performing tasks that are new or that the RN does not feel competent performing.
  • Attend and participate in continuing education programs designed to keep staff abreast of changes in the profession, changes in company policies/procedures, and completion of all annual mandatory education.
Requirements:
  • Credentials as a Licensed Practical Nurse with a current state license in good standing for the state in which work is performed.
  • Valid, current BLS/CPR and First Aid certification.
  • Must be at least 21 years of age.
  • Training and work experience in a pediatric setting preferred.
  • Meets the qualifications of a Child Care Supervisor as described in 55 Pa. Code Chapter 3800.
