Position Overview
We are looking for a compassionate and skilled Registered Nurse (RN) Case Manager specializing in Hospice care to become part of our dynamic healthcare team. This full-time, permanent role is essential for providing exceptional holistic care to our patients.

Key Responsibilities
  1. Deliver thorough nursing care to patients.
  2. Collaborate with the interdisciplinary team to create and implement personalized patient care plans.
  3. Regularly evaluate patient conditions, monitor vital signs, and modify care plans as needed.
  4. Offer emotional support and guidance to patients and their families, assisting them in navigating end-of-life choices.
  5. Coordinate with physicians, therapists, and other healthcare professionals to ensure cohesive care delivery.
  6. Educate patients and families on disease management, self-care practices, end-of-life care, and bereavement.
  7. Accurately document patient information, care provided, and any changes in condition.
  8. Ensure adherence to all hospice regulatory requirements at both federal and state levels.
  9. Participate in an on-call rotation to provide 24/7 patient support.

Qualifications
  1. Current and unrestricted Registered Nurse (RN) license to practice.
  2. A minimum of 2 years of nursing experience, with at least one year in hospice or palliative care preferred.
  3. Experience in case management and the creation of individualized care plans.
  4. Strong clinical nursing skills and a comprehensive understanding of hospice principles.
  5. Excellent communication and interpersonal skills to provide comfort and support.
  6. Ability to work independently, manage time effectively, and prioritize tasks.
  7. Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.
  8. Current CPR certification.
