Key Responsibilities:
  • Provide personalized support and care to individuals with complex needs
  • Develop meaningful relationships with participants and their families
  • Collaborate with a multidisciplinary team to create and implement individualized plans
  • Assist with daily living tasks, such as personal hygiene and medication management
  • Support participants in achieving their developmental goals, including budgeting, exercise, and nutrition

Requirements:
  • High school diploma or equivalent
  • 6 months of experience in human services, direct care, or care coordination
  • Strong communication, adaptability, and teamwork skills
