Job Summary

The Licensed Practical Nurse (LPN) at Christ's Home is responsible for providing high-quality care and services to residents in accordance with established policies and procedures. This role requires a strong commitment to delivering exceptional customer service and promoting a warm and welcoming environment.

Key Responsibilities
  • Supervise and coordinate the care of residents, ensuring their physical and emotional needs are met.
  • Develop and implement individualized care plans in collaboration with the Personal Care Nurse Manager and other healthcare professionals.
  • Provide education and support to residents and their families on health-related topics and community resources.
  • Maintain accurate and detailed records of resident care and services provided.
  • Collaborate with the care team to ensure seamless transitions and continuity of care.
Requirements
  • Graduation from an accredited practical nursing program.
  • Current CPR certification.
  • Active, unencumbered Practical Nursing license in the State of Pennsylvania.
  • One (1) or more years of experience in personal, assisted living, or long-term care.
  • Supervisory experience preferred.
  • Ability to lift a minimum of 50 pounds and perform physical tasks as required.
