Case Manager RN

At Mission Regional Medical Center, we are committed to delivering quality healthcare to our community. As a Case Manager Registered Nurse, you will play a vital role in ensuring the seamless delivery of care to our patients.
Key Responsibilities:
- Conduct comprehensive admission and concurrent reviews of medical records to ensure medical necessity, intensity of service, and severity of illness.
- Develop and implement individualized care plans to promote cost containment and quality outcomes.
- Collaborate with healthcare providers to integrate care and achieve a holistic approach to patient management.
- Provide education and support to patients and families to promote self-management and empowerment.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Minimum 5 years of experience as a Registered Nurse with a current state license.
- Minimum 5 years of experience in case management, discharge planning, or nursing management.
- Basic to intermediate computer skills.
- Behavioral Violence Prevention Training within 3 months of hire and maintenance of current certification.
