Job Title: Advance Care Planning Nurse

Cottage Health is seeking a dedicated and compassionate Advance Care Planning Nurse to join our team. In this role, you will play a vital part in supporting our Advance Care Planning Program, focusing on serving our Spanish-speaking population.

Key Responsibilities:
  • Provide advance care planning facilitation to patients and families, ensuring their needs are met and their voices are heard.
  • Process advance care documents received from community members, ensuring timely and accurate processing.
  • Develop and present education to hospital staff and medical providers on advance care planning, advance care documents, and Physician Orders for Life Sustaining Treatment (POLST).
  • Attend community meetings regarding advance care planning, fostering collaboration and awareness.
Requirements:
  • California RN license
  • BSN
  • 1+ years' experience in serious illness or hospice care, including end-of-life discussions and advance care planning training.
  • Fluent in Spanish, with the ability to obtain interpreter certification within one year of assuming the role.
  • ELNEC certification and POLST training preferred.
