Healthcare Operations Lead

The Healthcare Operations Lead plays a pivotal role in facilitating the transition of patients through the acute episode of care. As a member of the multidisciplinary team, this individual actively collaborates with internal and external resources to ensure seamless patient movement along the continuum of care.

Key Responsibilities:
\
1. Care Coordination/Transition Planning: The Healthcare Operations Lead proactively assesses patient acuity and transitional needs, collaborating with the Care Management team to address barriers and obtain necessary resources for timely discharge.\
\
2. Partners with the Interdisciplinary team to facilitate placement in post-acute settings, arrange DME, and other services as ordered.\
\
3. Identifies suitable consultative services to enhance patients' transitional plans.\
\
4. Encourages patient and family participation in care team decisions.\
\
5. Collaborates with post-acute services, care managers, and PCPs to ensure a smooth transition back to the home environment.\
People Management:
\
1. Assists in adjusting daily assignments based on staffing and caseload acuity.\
\
2. Demonstrates knowledge of NAH and department policies, working with leadership to ensure compliance with best practices.\
\
3. Establishes and maintains mutual trust and respect with staff.\
\
4. Fosters an environment that motivates and empowers staff, supporting department communication lines.\
\
5. Works with employees to achieve skill, educational, and performance goals.\
Resource Management:
\
1. Evaluates the appropriateness of care delivery in the inpatient setting, communicating discrepancies with the medical team.\
\
2. Facilitates care management in outpatient settings.\
\
3. Takes a leadership role in identifying opportunities to reduce financial and clinical risk, analyzing resource consumption outcomes.\
\
4. Assertively manages resource utilization while navigating patient movement along the continuum of care.\
Salary:
\
$85,000 - $110,000 per year, depending on experience.\
Requirements:
\
A Bachelor's Degree or Master's of Social Work is required; a Master's Degree in Nursing is preferred.\
\
An RN license in the State of Arizona is required; an LMSW or LCSW is preferred.\
\
Case Management Certification (ACMA, CMSA, CCMC) is also preferred.\
\
At least 1 year of experience in acute care management is preferred.\
