We are seeking a skilled Case Manager to join our team at HousingPlus, Inc. in Brooklyn. The successful candidate will work with individuals and families experiencing homelessness, mental illness, and substance use disorder to provide supportive services and help them achieve housing stability and independent living.
Key Responsibilities- Conduct initial intakes and assessments to identify client needs and develop individualized service plans
- Provide case management, counseling, and support services to clients, including referrals to community resources and agencies
- Collaborate with providers to ensure compliance and coordinate services
- Escort clients to appointments as needed and assist with achieving goals of self-sufficiency and independent living
- Maintain accurate and detailed case notes and data in AWARDS database
- Bachelor's degree in Human Services or related field
- Minimum 2 years' experience working with families around homelessness, mental illness, and substance use disorder
- Excellent organizational, interpersonal, and writing skills
- Proficient computer skills, including Microsoft applications and Google Suite
- Competitive salary: $45,000 - $47,000
