Job Summary:
We are seeking an experienced Hospice Office Nurse - LPN to join our team at Liberty Health. As a key member of our hospice care team, you will play a vital role in ensuring the highest level of care for our patients and their families.
Responsibilities:
- Coordinate the transfer of patients from home health services to palliative or hospice care, ensuring uninterrupted medical care.
- Assist in coordinating the transfer of patients residing in a skilled nursing facility (SNF) or discharging from a hospital setting to palliative or hospice services.
- Provide patient care, utilizing a variety of skills and necessary coordination of services.
- Complete accurate and timely documentation as required by policy and procedure.
- Assist in the management of patient care supplies and assist the Hospice Registered Nurse with medication reconciliation and pharmacist recommendations.
- Assist in the management of Medicare Part D medications.
Requirements:
- Current LPN licensure within the state of branch location and two years of clinical nursing experience, preferably one year in home health/hospice setting.
- Successfully completed nursing education from an approved school of nursing.
- Ability to independently organize work and provide effective healthcare services to hospice patients.
- Effective communication skills (oral and written) and CPR certification.
- Minimum of twelve hours of continuing education in a twelve-month period.
