Nurse Practitioner

We are seeking a compassionate and skilled Registered Nurse to join our team at Adoration Home Health and Hospice. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to ensure our patients receive the best possible care.
Responsibilities:- Provide safe and timely patient care using evidence-based nursing practices and best practices as defined by Adoration's quality assurance team.
- Participate in weekly team meetings to coordinate patient care and ensure seamless communication among team members.
- Develop and implement individualized treatment plans to facilitate patient rehabilitation and achieve optimal outcomes.
- Provide medication instruction, monitoring, and administration as ordered by the patient's Primary Care Physician.
- Conduct patient assessments and develop care plans in collaboration with the interdisciplinary team.
- Supervise Licensed Practical Nurses and Home Health Aide staff as needed.
- Document patient care and progress in a timely and accurate manner, adhering to state and federal regulations.
- Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the applicable state.
- One year of nursing practice in a patient care setting, with experience in home health, geriatrics, or related settings preferred.
- Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy.
- Current CPR certification.
