Position Overview: Collaborate with the Integrated Team to provide support and guidance to program participants, helping them navigate their unique pathways toward achieving individualized mental health recovery objectives.
Key Responsibilities:
- Manage the coordination and documentation of Intake, Assessment, and Treatment Plans, including the Self-Sufficiency Matrix and other evaluation tools. Make informed decisions regarding substance use.
- Conduct meetings with participants to evaluate substance use and addiction, assessing risks and impacts, and determining stages of change to create substance abuse formulations that inform Treatment Plans.
- Work collaboratively to recommend suitable interventions, which may involve brief interventions by the Substance Abuse Counselor or Recovery Coach, or referrals to additional treatment or self-help programs such as AA. Educate and model recovery strategies for staff and recovery coaches, while reviewing Evidence-Based Practices related to substance use.
- Partner with the team to implement Risk Management and Quality Improvement initiatives aimed at enhancing community tenure, stabilization, and improving clinical and recovery outcomes.
- Maintain a caseload of up to 15 participants, providing individual, family, and group psychotherapy focused on substance use, and documenting clinical work in electronic medical records. Complete intake assessments, service notes, and treatment plan reviews as necessary.
- Perform additional duties as assigned by the Clinical Team Leader, Contract Director, or their designee.
- Master's degree in a relevant field such as clinical psychology, counseling, medicine, psychology, psychiatric nursing, rehabilitative counseling, or social work.
- Licensed Substance Abuse Counselor (LADC I or II).
- At least one year of supervised experience in substance abuse counseling.
- Preferred experience working with individuals in mental health recovery.
- Proficient in computer skills.
- Possession of a vehicle, valid driver's license, and MVR on file.
- Salary range of $55,000 - $61,000, based on qualifications and experience.
