Job Summary

We are seeking a highly motivated and experienced Home Health Business Development Liaison to join our team at Choice Health at Home. As a key member of our business development team, you will be responsible for establishing and maintaining strong relationships with healthcare professionals, hospitals, and other key stakeholders to promote our home health care services.

Key Responsibilities
  • Establish new referral relationships and present our compelling care model to ensure Choice Health at Home becomes the preferred home health care provider.
  • Provide counsel to our agency on community needs, program development, and personnel and financial requirements.
  • Collaborate with our interdisciplinary team to support safe and effective patient care.
  • Develop and maintain policies specific to new programs and ensure compliance with local, state, and federal laws.
  • Meet and/or exceed admission goals on a monthly basis.
Requirements
  • Minimum of 2 years home health or hospice sales experience.
  • CRM software experience preferred.
  • Valid driver's license and auto insurance.
  • HomeCare HomeBase experience preferred.
