About the Role
Key Responsibilities:
- Provide clinical care coordination and case management services to assigned patients
- Collaborate with healthcare providers and staff to develop and implement individualized care plans
- Ensure timely and effective communication with patients, families, and healthcare teams
- Participate in quality improvement initiatives to enhance patient outcomes and experiences

Requirements
- Current licensure as an LPN in New York State
- 2+ years of experience in clinical care coordination or case management
- Strong clinical knowledge and skills, with a focus on LGBTQ+ healthcare
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
