Job Summary:

Oversees daily operations alongside management and leadership teams to ensure the achievement of clinical objectives and high-quality patient care for designated home care and/or hospice services.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Directs, educates, and supervises nursing and support personnel during assigned shifts, making patient and staff assignments. Contributes to performance evaluations of team members and serves as a resource for staff. Conducts patient visits as necessary.
  • Manages patient flow within the designated area and provides support during patient emergencies. Acts as clinical manager during operational hours, as assigned.
  • Coordinates patient care and referrals, ensuring continuous assessment of patient needs and the development, implementation, and updating of individualized care plans. Evaluates the effectiveness of patient care and identifies issues affecting desired outcomes.
  • Addresses complaints regarding patient care and manages them through established protocols.
  • May serve as the designated manager and/or alternate administrator of a Medicare provider number during operational hours as assigned. Upon obtaining written authorization, may be designated as an alternate administrator, reviewing obligations and receiving appropriate orientation.
  • Provides support and coordination for patient emergencies and leads incident command for internal or external disasters until relieved by an assigned administrative representative.
  • May assist in hiring, training, directing, developing, and evaluating staff.
  • Maintains a constant state of alertness and operates in a safe manner.
  • Performs additional duties as assigned.

EDUCATION

  • Graduate of an accredited nursing program or equivalent educational background for licensing

EXPERIENCE

  • Minimum of two years' experience as a registered nurse, demonstrating progressive leadership responsibilities

PHYSICAL REQUIREMENTS

  • Frequent lifting/carrying and pushing/pulling objects weighing up to 25 lbs.
  • Frequent sitting, standing, walking, reaching, and repetitive movements of the feet/legs and hands/arms.
  • Regular use of vision and depth perception for near (20 inches or less) and far (20 feet or more) distances, including color identification.
  • Frequent use of hearing and speech for oral communication, including the ability to hear alarms and malfunctioning equipment.
  • Regular keyboard use and data entry.
  • Occasional bending, stooping, kneeling, squatting, twisting, and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing between 25-50 lbs.
  • Rare climbing.
  • Frequent driving.

REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS

  • Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA)
  • And
  • Registered Professional Nurse (RN) - State Department of Financial and Professional Regulation
