About the Role

Communicare is seeking a dedicated and compassionate Direct Care Worker to join our team. As a Direct Care Worker, you will play a vital role in providing support and care to individuals with serious mental illness, children with serious emotional disturbances, individuals with substance abuse disorders, and individuals with intellectual and developmental disabilities.

Key Responsibilities
  • Provide direct care and support to individuals in the Supervised Living program, including assistance with daily living activities, socialization, and community integration.
  • Develop and implement individualized care plans to promote independence, social development, and skill acquisition.
  • Transport clients to various activities, appointments, and community events.
  • Collaborate with the Communicare Treatment Team to ensure comprehensive care and support.
  • Perform other duties as assigned by the Supervised Living Supervisor or other designated supervisor.
Requirements
  • High school diploma or GED.
  • Ability to perform the responsibilities of the job, including CPI certification or completion of CPI training.
  • Ability to relate positively to individuals served through the Supervised Living program and their families.
  • A current valid driver's license, automobile insurance, and an insurable driving record must be maintained continuously throughout employment.
