We are seeking a highly skilled and compassionate Director of Social Services to join our team at Willowbrook Manor. As a key member of our healthcare team, you will play a vital role in ensuring the well-being and quality of life for our residents.

Responsibilities
  • Supervise and guide our social workers in providing exceptional care and services to our residents.
  • Collaborate with facility staff to promote resident advocacy, protection, and rights.
  • Conduct admissions, social services evaluations, and provide referrals as needed.
  • Assist residents with financial and legal matters, such as applying for pensions or referrals to lawyers.
  • Develop and implement discharge plans to ensure safe and successful transitions for our residents.
  • Provide counseling services and psychiatric medication management as indicated.
  • Ensure accurate and timely documentation in MDS care plans.
Qualifications
  • Minimum of Bachelor's Degree in Social Work; Master's Degree preferred.
  • One year of supervised social work experience in a healthcare setting.
  • Current Social Work license from the state of MI.
