Family Medicine and Palliative Care Clinician

We are seeking a dedicated Family Medicine and Palliative Care Clinician to join our team at Gentiva Health Services.
About the Role:
- To provide comprehensive, patient-centered care to individuals and families, addressing their physical, emotional, and spiritual needs.
- To work collaboratively with healthcare professionals, patients, and families to develop individualized care plans that prioritize comfort, dignity, and quality of life.
- To educate patients, families, and caregivers about disease management, symptom control, and palliative care principles.
- To coordinate care with other healthcare providers, ensuring seamless transitions and optimal outcomes.
About You:
- Masters of Science in Nursing from an accredited school of nursing, or equivalent as allowed under state law.
- Minimum two years of experience as a Nurse Practitioner, preferably in family medicine and palliative care.
- Active RN license and NP certification.
- Ability to work autonomously and collaborate effectively within interdisciplinary teams.
