At Conviva, we are dedicated to delivering exceptional primary care services to our patients. As a Primary Care Physician, you will be part of a dynamic team that prioritizes patient-centered care, fostering a collaborative environment where healthcare professionals work together to provide comprehensive care.
**Key Responsibilities:**
Evaluate and treat patients in accordance with established standards of care.
Collaborate with multidisciplinary teams to coordinate patient services, including specialty referrals, hospital coordination, and durable medical equipment.
Participate in daily huddles to ensure seamless communication and care coordination.
Assist in setting a tone of cooperation and professionalism within the practice.
Complete medical record documentation in a timely manner, working with quality-based coders to optimize coding specificity.
Follow policy and protocol defined by Clinical Leadership.
Meet with Regional Medical Director to discuss quality of care, review outcome data, and address policy and procedure issues.
Participate in growth opportunities for new or existing services within the Center.
Participate in the local primary care 'on-call' program as needed.
Ensure personal compliance with licensing, certification, and accrediting bodies.
Spend 100% of your time clinically focused on direct patient care.
**Requirements:**
Current and unrestricted medical license or willingness to obtain a medical license in the state of practice.
Graduate of an accredited MD or DO program.
Excellent verbal and written communication skills.
Ability to demonstrate a high level of skill in interpersonal relationships and communications with colleagues and patients.
Willingness and ability to learn and adapt to practice in a value-based care setting.
Superior patient/customer service skills.
Basic computer skills, including email and EMR.
**Preferred Qualifications:**
Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
Active and unrestricted DEA license.
Medicare Provider Number.
Minimum of two to five years directly applicable experience.
Experience managing Medicare Advantage panels of patients with understanding of best practices in coordinated care environments.
Knowledge of Medicare guidelines and coverage.
Knowledge of HEDIS quality indicators.

**Pay Range:**
The compensation range below reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, certifications, etc. $199,400 - $298,300 per year. This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
