About the Role

At Suncrest Hospice, we are committed to delivering exceptional care and service to our patients and their families. As a Certified Nursing Assistant/Home Health Aide, you will play a vital role in our team, working under the direction of a Case Manager/DON to provide resident care to assigned residents in accordance with resident assessments, care plans, and nursing management directives.

Key Responsibilities
  • Provide resident care to assigned residents, adhering to resident assessments, care plans, and nursing management guidelines.
  • Collaborate with licensed care staff to coordinate proper types of care to residents, ensuring seamless delivery of services.
  • Strictly follow all Company nursing policies and procedures, as well as safety guidelines, to maintain a safe and healthy environment for residents and staff.
Requirements
  • Strong commitment to improving quality of life while partnering with community organizations in patient care.
  • Ability to work well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and STNA leadership.
  • Eager to gain higher-level opportunities, such as certifications and further education.
  • Willingness to work independently and on your own from a day-to-day basis, with strong support from a patient care team.
  • Reliable transportation, valid driver's license, and auto insurance.
  • Certified Nursing Assistant (CNA) certification in the state(s) of practice.
  • Home Health Aide (HHA) certification in the state(s) of practice.
  • CPR certification.
