Job Summary

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Physical Therapist to join our team of skilled home healthcare providers. As a Physical Therapist, you will play a vital role in providing quality care to our patients in their homes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement individualized treatment plans in consultation with physicians and revise as necessary.
  • Collaborate with the interdisciplinary team to develop and implement client care plans.
  • Teach, supervise, and counsel patients and their families on physical therapy programs.
  • Supervise Physical Therapist Assistants and communicate with other healthcare professionals as needed.
  • Document patient progress and maintain accurate records.
Requirements
  • Graduate of an accredited Physical Therapy program or equivalent education.
  • Licensed to practice as a PT in the State of Florida.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • At least one year of experience as a Physical Therapist.
  • Home healthcare experience is preferred but not essential.
  • Electronic medical record experience is preferred but not essential.
  • Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting.
