Job Summary:

Regal Medical Group is seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in providing emotional support and guidance to our patients and their families, helping them navigate the complexities of the healthcare system.

Key Responsibilities:
  • Conduct thorough assessments of patients' social and emotional needs, identifying areas where support is required.
  • Develop and implement individualized care plans to address patients' needs, working closely with healthcare teams to ensure comprehensive care.
  • Provide emotional support and guidance to patients and their families, helping them cope with illness, injury, or other health-related challenges.
  • Connect patients with community resources and services, such as counseling, support groups, and home care services.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Document patient interactions and progress in a timely and accurate manner.
Requirements:
  • Master's degree in Social Work (MSW) or related field.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW).
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong assessment and problem-solving skills, with the ability to think critically and creatively.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Knowledge of community resources and services, with the ability to connect patients with relevant support.
