Job DescriptionJob Title: Home Health Registered Nurse Case Manager
Job Summary: We are seeking a dedicated and experienced Home Health Registered Nurse Case Manager to join our team. As a Home Health Registered Nurse Case Manager, you will play a pivotal role in coordinating and providing comprehensive healthcare services to our patients in their homes.
Responsibilities:
- Conduct thorough patient assessments to develop personalized care plans.
- Administer skilled nursing care in accordance with physician's orders and established protocols.
- Monitor and document patient progress, ensuring accurate and timely charting.
- Educate patients and their families on disease management, treatment plans, and self-care techniques.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate and ensure seamless care transitions.
- Participate in case conferences and interdisciplinary team meetings to review patient progress and adjust care plans as needed.
- Comply with all regulatory guidelines, including OASIS assessment and documentation.
Qualifications:
- Active Registered Nurse (RN) license in the state of Florida.
- Minimum of 2 years of experience in home health care.
- Proficiency in OASIS documentation.
- Ability to exercise initiative and independent judgment.
- Strong clinical and assessment skills.
- Excellent communication and interpersonal skills.
- Proficient computer skills for accurate documentation and electronic medical records.
- Reliable transportation and valid driver's license.
