Clinical Nurse Coordinator- Poughkeepsie, NY

We are seeking a highly skilled Clinical Nurse Coordinator to join our team at Nuvance Health. The successful candidate will be responsible for coordinating clinical and administrative activities related to the outpatient physician's practice, ensuring high-quality, safe patient care for a high-risk disease-specific population.
Key Responsibilities- Assist supervising physicians with patient visits, including physical assessments and health histories, and coordinate care with nursing staff and other providers.
- Respond to and refer incoming patient care-related phone calls, instructing patients and caregivers on medications and treatment instructions.
- Collect and track clinical data, including laboratory, radiology, and pathology results, to ensure receipt of ordered tests.
- Review abnormal data with primary physicians, coordinating the need for specific interventions, such as further testing, medication implementation, or referrals to other providers or agencies.
- Participate in performance improvement activities designated by the Chief of Oncology or Practice Manager to ensure oncology patients receive best-quality care.
- Assist in training newly hired nurses in the care, triage, and complete assessment of quality and scope of practice, supervising nurses and medical assistants to ensure excellent care is provided to patients.
- Collaborate with the multidisciplinary healthcare team to coordinate services for oncology/hematology patients, including referring and primary care physicians, radiation therapy, inpatient treatment, palliative care, survivorship programs, hospice, home care, social work, nurse and financial navigators, dietary, respiratory agencies, and community support programs.
- Work with the AVP of Nursing for NHMP or designee to ensure annual and departmental competencies are completed, identifying areas of education for practice nurses and medical assistants.
- Create educational materials for nurses/MAs as needed.
- Fulfill all compliance responsibilities related to the position, maintaining and modeling Nuvance Health's Foundational Values (Personal, Imaginative, Agile, Connected).
- Registered Nurse (RN) license in the State of NY.
- Three years of office nursing experience.
- BSN required; Oncology Nursing or surgical oncology nursing background required.
This position will be based at the Poughkeepsie-Dyson Center, 45 Reade Place, with a standard work schedule of 40 hours per week, Monday through Friday, 8:00 am - 5:00 pm, with flexibility to accommodate the needs of the practice.
