About the Position

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of our members' homes, identifying potential health risks and developing personalized care plans to improve their overall well-being.

You will work collaboratively with our team of clinicians and care managers to provide seamless care coordination and support. Your expertise and compassion will make a direct impact on the health and well-being of our members, and you will be rewarded with competitive compensation and flexible scheduling.

Responsibilities
  • Conduct comprehensive health assessments in members' homes, identifying potential health risks and developing personalized care plans
  • Work collaboratively with clinicians and care managers to provide seamless care coordination and support
  • Develop and implement care plans to improve members' overall well-being
  • Provide education and support to members and their families to promote healthy behaviors and self-management
  • Collaborate with primary care providers to ensure seamless care coordination and communication
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
Travel Requirements

This position requires frequent travel to meet with members in their homes. You must have a valid state driver's license and be able to drive a car. Proof of adequate automobile insurance coverage is also required.
