About the Job

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Betsy Ross Nursing & Rehab Center. As a RN Case Manager, you will be responsible for overseeing the operation of the nursing department and ensuring that residents receive high-quality care.

Key Responsibilities:
  • Organize and supervise assigned unit activities and staff assignments
  • Identify and respond to patient needs in a timely and appropriate manner
  • Monitor and document vital signs, treatments administered, and patient responses
  • Administer medications and treatments as needed
  • Communicate and direct services to appropriate team members, residents, family, or medical staff as needed
  • Collaborate with interdisciplinary teams, physicians, and other vendors/services
  • Perform RN assessments to residents' post-incidents or change in condition
  • Lead and direct emergency situations
  • Maintain accurate nursing reports, staffing sheets, and census rosters
Requirements:
  • Current and good standing RN License in State of New York
  • Experience with geriatric/short-term rehab/Alzheimer's patients is desirable
  • Progressive nursing experience, supervisory experience preferred
  • Dedicated to providing extraordinary, quality, compassionate care to each resident
  • Genuinely interested in helping ill, injured, disabled, and elderly
  • Have patience and willingness to assist any residents and co-workers
  • Have excellent communication, interpersonal, customer service, and organization skills
  • Able to work well under pressure, flexible, and will work harmoniously with all staff
  • Covid-19 vaccinations are encouraged but not mandated
