Job SummaryThe Primary Care Physician will play a vital role in our team, performing comprehensive assessments, coordinating care plans, and collaborating with our interdisciplinary team to ensure effective care coordination. This is an exciting opportunity to design and implement care strategies that make a meaningful difference in the lives of our participants.
Key Responsibilities- Conduct in-person comprehensive assessments and develop care plans for each participant
- Collaborate with our interdisciplinary team to ensure seamless care coordination
- Provide direction and support to Nurse Practitioners and other team members
- Oversee care of participants in nursing homes and hospitals, including admits and discharges
- Participate in rotating night and weekend call
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
- Doctorate Degree
- Board Certification – Internal Medicine or Family Medicine or Emergency Medicine or Geriatrics (preferred)
- Active California Physician License without Restrictions
- Active Drug Enforcement Administration (DEA)
- Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
- Reliable means of transportation
- State CDS Certification and DEA Registration
- Competitive salary range: $250,000 - $280,000 / year + Bonus + Equity
