Job SummaryThe Mental Health Social Practitioner will be responsible for implementing social practice principles to promote member engagement, social design, and relationship development. You will work closely with members to identify and address barriers to recovery, such as social isolation, poor self-esteem, and anxiety.
Key Responsibilities- Develop and implement individualized plans to support members in achieving their goals
- Provide training and support to members in vocational training and employment opportunities
- Assist members with entitlements and benefits, including food stamps, SSI/SSDI, and Medicaid/Medicare
- Facilitate supported housing applications and work to find appropriate housing for members
- Advocate for members' well-being, safety, and livelihood
- Undergraduate degree or equivalent combination of training and experience required
- Graduate degree or equivalent preferred
- 1-3+ years' experience working with individuals with serious mental illness and/or hands-on experience working in a community setting preferred
