Responsibilities- Implement and monitor patient care plans, ensuring the highest level of care for our patients.
- Coordinate with the healthcare team to provide well-coordinated patient care, ensuring seamless transitions and optimal outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure accurate and timely information exchange.
- Assess and coordinate patient discharge planning needs, ensuring a smooth transition to post-acute care.
- Compile data and track outcomes to meet grant specifications, driving quality improvement initiatives.
- Apply evidence-based practice and scientific knowledge to nursing practice, ensuring patient care is delivered based on age-specific needs and clinical requirements.
- Work collaboratively with the healthcare team to ensure compliance with grant requirements and regulatory standards.
- Perform other duties as assigned, contributing to the overall success of our team.
- Graduate of an accredited school of nursing or equivalent education for licensure.
- One year of registered nurse experience, with a focus on critical care or trauma nursing.
