We are seeking a highly skilled Nurse Practitioner to join our team at Abode Care Partners. This role will provide evidence-based medical care to seniors and patients with complex needs in their home or assisted living facilities.

About the Role

This is a unique opportunity to make a positive impact on people's lives while advancing your medical skills. As a House Calls Nurse Practitioner, you will be responsible for diagnosing illnesses, collaborating with healthcare professionals, and ensuring accurate patient records.

Responsibilities
  • Medical Evaluation and Treatment: Conduct thorough medical evaluations, diagnose illnesses, and develop treatment plans to meet the unique needs of each patient.
  • Patient Collaboration: Work closely with caregivers, family members, and other healthcare professionals to ensure comprehensive care and effective communication.
  • Record Keeping: Maintain accurate and up-to-date patient medical records, documenting all interactions and treatment plans.
Requirements
  • Licensure and Certification: Active state Nurse Practitioners license, Current Nurse Practitioner National Board Certification, and Active state DEA licensure.
  • Education: Master's degree in Nursing (MSN).
  • Experience: Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Skillset: Strong interpersonal communication skills, excellent time management skills, and proficiency in medical instruments and equipment.

Estimated Annual Salary: $110,000 - $125,000
