Job Summary:

We are seeking an experienced Advanced Practice Provider to join our team at OSF HealthCare. As a Geriatric Care Provider, you will provide primary care to participants in our PACE Program, a Medicare and Medicaid program that helps people meet their health care needs in the community.

Responsibilities:

Perform physical assessments of new PACE Enrollees and semi-annual reassessments of participants
Develop and implement appropriate plans of care for program participants
Evaluate participant physical complaints and provide appropriate treatment
Provide participants and caregiver teaching and education
Function as a member of the Interdisciplinary Team (IDT)

Requirements:

Master's degree or higher in nursing or physician assistant studies from an accredited program
Current Illinois license as a nurse practitioner or physician assistant
Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
Minimum of two years of experience as an APP in geriatrics or a related field
Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
Proficiency in electronic medical record (EMR) systems
