Job Title: RN Case Manager - High Risk Family Nurse

Coordinated Youth and Human Services (CYHS) is seeking a highly skilled and compassionate Registered Nurse to join our team as a RN Case Manager - High Risk Family Nurse. In this critical role, you will provide high-quality nursing care and support to high-risk pregnant and postpartum individuals and their infants, with a focus on reducing maternal and infant morbidity and mortality rates.

Job Summary:

The RN Case Manager - High Risk Family Nurse will provide comprehensive nursing assessments, interventions, and service coordination to improve the health, social, educational, and developmental needs of high-risk families. This includes providing home visits, phone contacts, and referrals to health-related and public assistance programs. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse groups of people.

Key Responsibilities:
  • Provide high-quality nursing care and support to high-risk pregnant and postpartum individuals and their infants
  • Conduct comprehensive nursing assessments and develop individualized care plans
  • Provide education and support to families on maternal and infant health, including nutrition, safe sleep practices, and mental health
  • Coordinate referrals to health-related and public assistance programs
  • Build and maintain strong relationships with community partners, including medical providers, social services, and community-based organizations
  • Participate in outreach and education activities to promote program awareness and enrollment
Requirements:
  • Registered Nurse (RN) license in the state of Illinois
  • Experience working with high-risk pregnant and postpartum individuals and their infants
  • Excellent communication and interpersonal skills
  • Ability to work effectively with diverse groups of people
  • Strong organizational and time management skills
  • Ability to maintain confidentiality and handle sensitive information
