We are seeking a compassionate and skilled Pediatric Home Health Nurse to provide high-quality medical care to children with complex medical needs in the comfort of their own homes. As a member of our interdisciplinary team, you will work closely with families, caregivers, and other healthcare professionals to develop and implement individualized care plans that promote optimal health and well-being.

Responsibilities:
  • Provide skilled medical care, including assessments, medication administration, and therapies, in accordance with the plan of care and physician's orders.
  • Collaborate with families, caregivers, and other healthcare professionals to ensure seamless communication and coordination of care.
  • Develop and implement individualized care plans that address the unique needs and goals of each child and family.
  • Provide education and support to families and caregivers to empower them to take an active role in their child's care.
  • Document all care activities and communicate with the healthcare team to ensure continuity of care.
Requirements:
  • Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure, without restrictions.
  • Valid driver's license and proof of auto liability insurance coverage.
  • CPR certification required but may be obtained after hire.
  • COVID-19 vaccine compliance.
  • Ability to lift, carry, push, or pull up to 50 pounds.
